FORMS
ADULTS -
Print enrollment form and return to office with
deposit or full Package amount and copy of
Instruction Permit. Be sure to call for
availability!
TEEN ADDITIONAL
HOURS/PACKAGES - Print enrollment form and
return to office with deposit or full Package amount
and copy of Instruction Permit. Be sure to call for
availability!
TEEN DRIVER
EDUCATION PROGRAM - Print enrollment form and
return to office with $100.00 deposit or full
payment. You are not considered enrolled until
payment and signed form are received. Call for class availability.
You will also need to provide Proof of Eligibility
on the first day of class to enable us to request a
“Letter of completion” from the Secretary of State
in order to obtain a license. (see below)
ELIGIBILITY FORMS
DRIVER EDUCATION
APPROVAL FORM - This form is provided for
students who have passed eight classes during the
previous two semesters (includes grade school). Copies of report cards can
also be accepted.
HOME SCHOOLED
PARENTAL CONSENT FORM- This form is for students
who are home schooled.
G.E.D. -
written documentation required showing proof of
enrollment or completion.
OTHER
HELPFUL FORMS
50 HOUR
CERTIFICATION -State form for recording 50 hours
of mandatory driving time.
PARENT - TEEN
DRIVING CONTRACT - Recommended by the Illinois
Teen Safety Task Force as a way to encourage safe
and responsible driving. Solely for use between
parent and teen.
AFFIDAVIT /
CONSENT FOR MINOR TO DRIVE -Gives written
consent for a 16-17 year old to drive if parent is
unable to accompany teen to Driving facility to take
test.
